It can be extremely challenging to chew food properly and pronounce certain words with an open bite, and the facial structure is typically affected by this condition. POKING WIRES OR METAL PIECES How to fix a poking wire on braces: If your braces wire pops out of place or you have a metal piece bothering you, grab some orthodontic wax. Orthodontic wax (also called dental wax) can be placed over the metal piece or poking wire to serve as a temporary cushion until you’re able to visit us. If a wire pops out of place, try to push the wire back into the bracket. Use sterilized tweezers or a clean pencil eraser to push the wire back into the bracket. You can also try to clip the loose section of the wire off with a clean pair of nail clippers, then cover the end of the wire with ortho wax. LOOSE OR FLOATING BRACKET ON BRACES If you have a floating bracket on your braces and it’s still attached to your wire, be extra careful not to swallow the bracket. Take some orthodontic wax, and place it over the floating bracket to temporarily attach it back to the tooth. This way, the bracket won’t bother your cheeks and you’re less likely to swallow the bracket. If you do happen to swallow your braces bracket, don’t panic. It happens! The swallowed bracket will likely go straight to your stomach with the rest of your food, and pass naturally. If you experience a severe stomach ache, breathing problems, or any unusual symptoms, go to the emergency room. How long can you go with a loose bracket? While a loose bracket is not an urgent situation, it does require your orthodontist’s attention as soon as possible. In other words, you don’t need to drop everything and hightail it to our office, but you should make it a priority to see us within the week! What is corrective jaw surgery? Surgical orthodontics, also known as orthognathic surgery and corrective jaw surgery, is a more intensive orthodontic treatment that serves to treat jaw irregularities, skeletal issues and severe misalignments. Through early orthodontic treatment, we can help to prevent the need for surgical orthodontics later in life. This is why it’s important that children receive their first orthodontic exam by the age of 7. If we determine that orthognathic surgery is necessary, we will refer you to an experienced surgeon. The surgery may take a few hours, and generally requires a two-week recovery period post-surgery. We advise that you take time off of work or school to allow for a complete recovery. Once your jaws have healed, Dr. Loeb will reassess your bite to ensure that the treatment was effective. You will typically undergo braces treatment after surgery, for the next 6 to 12 months depending on your case. After your braces treatment, you’ll wear a custom retainer to stabilize your teeth and support them in their new positions. Are there any risks associated with jaw surgery? Like any major surgery, there are risks to orthognathic surgery. Wearing your retainer is the final (and most important) phase of your orthodontic treatment in Oakland. It’s up to you to maintain your results through proper retainer wear and care. What does a retainer do? Once active orthodontic treatment is complete, your teeth are susceptible to orthodontic relapse, or reverting back to the original pre-treatment position. How long will I wear a retainer after braces or clear aligners? Once active orthodontic treatment is completed, you will begin the retention phase. Removable retainers are typically worn nights only. Because teeth can shift at any age, we recommend the use of retainers for as many years as you do not want your teeth to move. Types of Retainers There are two types of retainers, removable retainers and fixed retainers. Removable retainers are similar to Invisalign clear aligners. They can be taken out as needed to eat, brush your teeth, and clean your retainer. Fixed retainers, or permanent retainers, are bonded to your teeth and cannot be removed without your orthodontist. Removable Retainers The main types of removable retainers are Hawley retainers and clear retainers. The Hawley retainer has metal wires with an acrylic center that sits in the roof of the mouth. A clear retainer is similar in concept to Invisalign or a bleaching tray. Made of clear plastic, it's a vacuum formed shell that covers the teeth. Fixed or Permanent Retainer (Bonded Wire) These retainers include a small metal wire, custom-designed to fit behind the teeth. They’re generally bonded to the back surface of the bottom teeth, but can also be used behind the top teeth depending on your condition. Typically, this wire stays in place forever to hold your bottom teeth in their beautiful alignment. The only exception is if your family dentist determines that your oral hygiene is suffering, which could lead to bone or gum problems. Keeping your teeth clean with a permanent retainer is essential. Retainer Cost You will receive one custom retainer as a part of your treatment plan. If you happen to lose or break your retainer, there will be a replacement fee. If you lose or damage your retainer, please let us know as soon as possible. We want you to have a new retainer as soon as possible to prevent your teeth from moving out of place. The rapid palatal expander (the most common type of palatal expander) is bonded to the upper molars on both sides of the top jaw, with a screw located in the center of the appliance. To activate the palatal expander, you’ll insert a special key to turn the center screw, which expands the device ever so slightly and widens the palate gradually over time. If your child requires a palatal expander, we’ll tell you everything you need to know about cleaning it, activating it and caring for it. TADs Temporary anchorage devices (TADs) are temporary dental implants embedded into the bone to create an “anchor” that assists with tooth movement. These appliances are generally used when certain teeth require shifting, and others don’t. TADs enable us to treat individual teeth with greater efficiency. These anchorage devices appear as tiny metal buttons, strategically implanted into an area of the jaw bone. A small wire or elastic may be attached to the TAD to connect it with the specific tooth (or teeth) in need of treatment. Teeth Separators or Spacers Spacers, also called separators, are small rubber bands that help to create the space necessary for braces to be placed. With braces, a metal band is placed around the back molars to anchor the arch wires in place. In cases where the molars are too close together to allow these metal bands to be placed, separators may be used. Separators are temporary appliances that are only worn for about one to two weeks before active treatment begins. Nance Appliance The Nance appliance is used to hold the upper molars in place, to prevent them from rotating or shifting forward. With this appliance, two metal bands are cemented around the first molars and a wire connects the back molars to an acrylic pad, located behind the two front teeth. Reverse-Pull Headgear (Orthodontic Facemask) The reverse-pull headgear (facemask) is an incredibly effective appliance used to promote proper alignment of the jaw bones for young kids. It works by pulling the upper jaw forward. This appliance treats underbite conditions that may otherwise require jaw surgery to correct. This appliance can be incredibly helpful for young patients, ages 7-10. When we place your braces on, we are simply bonding brackets to your teeth, and attaching your archwires to your brackets. This won’t cause you any pain. The first week you have braces, you may experience some minor soreness as your teeth and gums are adjusting to the support of your braces. You can take whatever over-the-counter pain reliever you normally would for a headache to counteract the soreness. This should subside after the first week or two. HOW LONG WILL MY TREATMENT PERIOD BE? This will depend on a few factors, including your overall oral health, the severity of your treatment, and your diligence in following directions and wearing your appliances (like rubber bands). On average, a full treatment can take anywhere from 12 to 36 months, although your case may be shorter or longer. The length of your treatment may change, depending on how quickly your teeth are shifting and whether you wear your appliances as instructed. CAN I PLAY SPORTS WITH BRACES? Yes, your orthodontic treatment won’t stop you from living your best life! As long as you wear a mouthguard when playing sports, you won’t have to sit on the sidelines during the season. With any type of braces, it’s incredibly important that you wear a mouthguard while playing sports. A mouthguard will protect you and other players from damage. WHAT CAN I EAT WITH BRACES? You can still eat at your favorite restaurants in Oakland! You’ll be able to eat tons of delicious snacks, meals and desserts with braces too. During the first week of your treatment, as your teeth and gums are adjusting, you may want to choose softer foods, like pasta, tuna, mashed potatoes, or soup.
